The breast augmentation (mammoplasty) is the most popular manipulation in plastic surgery. According to the experts, more than 100 000 breast implantations are made in the world each year. The main reasons that encourage women to seek help from a plastic surgeon are the small breast, changes caused by childbirth and breastfeeding and breast asymmetry.
The first question of the patients that apply to the clinic is: "Is the implant installation safe, does the surgery provide the overall influence on the body?"
To determine whether there is a connection between breast cancer and silicone implants, a group of women who were subject to the breast implantation were compared to the same age equal group of women who did not increase breast with silicone implants. These groups were monitored regarding the frequency of cancer pathology. The surprising result showed that the incidence of breast cancer pathology was higher in the group having no implants. I repeat - the result of research conducted in a strictly scientific framework, shows that the risk of breast cancer for women with implants is lower rather than higher. Also, studies conducted at the Mayo Clinic located in the United States have shown that there is no connection between silicone implants and autoimmune diseases.
Today, all the implants used for augmentation breast surgery have a solid shell with a smooth or textured surface. Implants can be round, anatomical, teardrop-shaped; they have a different projection as well. In each case, implants of a certain size, shape and projection are chosen after the assessment of the covering tissues, forms of bone support, width of the breast base and the individual wishes of the patient. Evaluation of all the parameters is needed to achieve the best aesthetic results.

The important point is good information awareness of patients before breast augmentation. You should have realistic expectations in relation to the result and should be familiarized about the possible risks, particularly the recovery period. All of the patients that are subject to the breast augmentation should undergo laboratory examination – the breast ultrasonography or mammography.
Augmentation breast surgery
The most preferred form of intraoperative anesthesia is a combination of general and local anesthesia. This eliminates all discomfort during mammoplasty and facilitates the state immediately after it.
There are three main access points to form a tissue pocket. These are the submammary access point (in the furrow under the breast), axillary access point (in the axillae) and transareolyarny access point (along areola edge). Submammary access point is the least traumatic and is used most frequently. It is an incision having a length of 3-4 cm that is used to form the required cavity under the interfacial band deep leaflet and under the pectoralis major muscle. Thus, the mammary gland remains on the top and does not suffer any trauma.
During the plastic surgery, the surgeon pays special attention to control of bleeding from the small blood vessels. Implants are installed in the formed cavity. Three rows of sutures are applied: on the band, on the fatty tissue, on the skin. Then the steristriks strips and aseptic bandage are applied to the wound, and compressive linen is put on immediately.
Post operation period
After the surgery, the patients stay in the clinic during the first day, and then they should come for bandaging and performing of check-ups by a doctor. The patients should wear a compressive uplift for 3-4 weeks. The sutures dissolve on their own in 10-12 days after mammoplasty..
It is too early to assess the resultant breast shape during the first bandaging. The breast looks flattened, the upper semi-sphere is filled excessively, and there is a swelling in the sternum area. The breast is elastic and painful moderately. Subjective discomfort usually disappears within 3-4 days, and it takes up to 1.5-2 months for the final formation.. During this time the band system and the muscles are stretched uniformly over the implant, the swelling decreases, and the implant goes down slightly. Breast becomes much softer and more flexible. Formation of dermal scarring ends in 10 - 12 months.
Possible complications
1. Hematoma (accumulation of blood in the formed cavity). Most often this complication is eliminated in a paracentetic way, but a re-examination of the implant cavity is being performed if necessary.
2. Capsular contracture. The fibrous capsule of connective tissue is formed around the implant; it separates the implant from the soft tissues. Sometimes the capsule can compact and squeeze the implant. As it reduces its volume, there is a compression of the implant. Such problems are caused by the individual reaction of the body; unfortunately, it is impossible to predict such problems before the surgery. This complication sometimes requires surgical treatment: the complete or partial removal of the capsule (capsule ectomy).
3. Hypertrophic or keloid scar. It arises in connection with the disposition of the body to form such scars. It is a rare case and requires a conservative treatment; however in some cases a surgical treatment is necessary./p>
